New collaborative care clinic in St. Stephen

Access to primary health care in Charlotte County is set to improve with the launch of a new collaborative care clinic in St. Stephen.
The provincial government has committed to establishing and supporting at least 10 collaborative care teams in 2025. The St. Stephen team is the ninth announced this year.
Approximately $3.6 million is being provided to the Horizon Health Network to establish a dedicated clinic that will expand primary care services for Charlotte County residents.
“Making sure New Brunswickers can get the care they need, when they need it, is our top priority,” said Premier Susan Holt. “We’re thrilled to support the expansion of the Charlotte County Collaborative Wellness Centre team and the dedicated health-care workers who are helping connect more people in St. Stephen with primary care close to home.”
The provincial investment will enable the team to move into a larger clinic space, recruit additional health-care providers to enhance primary care capacity, and welcome more patients to the practice.
The centre’s collaborative care team was created in February and has since expanded to 10 health-care professionals. It has attached 310 patients who previously did not have a primary care provider. The centre has also opened an access clinic, in partnership with local physicians, to serve patients with non-urgent needs. More than 2,000 people have received support through this service since its launch.
The St. Stephen team now includes one physician, three nurse practitioners, two registered nurses, two licensed practical nurses, one respiratory therapist and one social worker.
“Collaborative care delivers stronger health care for patients and enhanced support for providers,” said Health Minister John Dornan. “This expanded team of primary care and allied health professionals in St. Stephen is a clear example of how we are strengthening our health system with team-based care.”
Primary care is one of six priority areas outlined in the provincial health plan, Caring for New Brunswick: Putting People at the Heart of Health Care.
Over four years, the government has committed to establishing and supporting at least 30 local collaborative care teams across the province.
Regional health authorities continue to work with health-care professionals to expand team-based care, tailoring each team to the needs of its community. Collaborative care teams have been announced for Campbellton, Moncton, Miramichi, Carleton North, Tantramar, Île-de-Lamèque, Fredericton, Edmundston and St. Stephen, with at least one additional team expected before year-end.
“The growth we’ve seen at the Charlotte County Collaborative Wellness Centre shows how team-based collaborative care is transforming primary health-care delivery in New Brunswick,” said Ashley Calvert, Horizon Health Network’s interim vice-president of community. “We look forward to building on this momentum, bringing more professionals into the team and ensuring Charlotte County residents have improved access to the primary care they need.”
